Position Summary:
Ensure that GWH associates receive services from Workforce Development (WFD), strategic partners, and other community resources consistent with the mission, identified outcomes and the associates' individualized plan that outlines a move toward retention and growth.
Responsible for a variety of roles in order to provide individual associates, and when warranted their families, comprehensive, "single point of contact" support as part of a seamless service delivery design. Services to the associate include life and work coaching, personal and career counseling, work assessments, mediation, coordination and advocacy with organizational and community resources, and providing case management with a goal of achieving self-sufficiency.
Essential Duties and Responsibilities:
- Accountable for the associates' full Individual Development Plan (IDP) cycle, including assessing, planning, implementing, monitoring and evaluating actions required to meet the associate's accommodation, retention and career growth needs.
- Effectively develop positive empowering relationships with the associates through coaching, motivating, and educating.
- Actively promotes GWH programs to create awareness and demonstrate the potential it has in career and life advancement.
- Be fluent in the evidence-based career navigation model and the specific career pathways within GWH.
- Be fluent in local community resources and programs and provide constant referrals to programs that foster continued growth of the Associate.
- Maintain working knowledge of educational and training opportunities located inside and externally in the community.
- Provide direct case management services in accordance with policies, procedures, and standards associated with GWH WFD to further the success of the associate through the use of Individual Development Plans.
- Document coaching, appointments, and activities with the associates through case notes in electronic case management systems.
- Coordinate with associates and pre-schedule meetings.
- Refer associates to outside sources or additional training services or social aids.
- Be able to establish a rapport with associates.
- Intervene in crises.
- Maintain accurate records.
- Travel to multiple locations as needed.
- Other duties as assigned by management.
- Demonstrate professionalism and good judgment and present a friendly, cooperative attitude.
- Ability to multi-task, make quick decisions, adapt to change, and work in a team
-oriented, fast-paced environment. - Adheres to all GWH Policies & Procedures, Loss Prevention & Safety Regulations, and GWH Guiding Principles.
- Must be punctual and have dependable attendance and maintain an appropriate appearance.
- Must conduct work in a safe manner; maintain good housekeeping standards; have all aisles and pathways to fire exits clear and free from clutter and keep work areas neat and clean.
- Adheres to local, state, and federal laws and regulations.
- A high school diploma or equivalent is required.
- A minimum of one (1) year of experience in case management is preferred.
- Extensive knowledge of community resources in the four sector areas: business, government, non-profit, and faith-based organizations.
- Experience as a facilitator, instructor or career coach.
- Good computer skills (proficient in Microsoft Office).
- Excellent written and verbal communication skills.
- Ability to communicate and interact well with a variety of personality types.
- Ability to analyze opportunities and problems, identify and evaluate alternatives, and develop effective approaches to address any issues.
- Ability to plan, implement and evaluate the achievement goals, objectives, and work plans.
- Must maintain a high level of confidentiality.
- Must have reliable transportation.
- Ability to sit, stand, bend, and reach.
- Ability to lift, push, pull, up to, 25 lbs.
