Description
Seeking part time LMSW as a Hospice Social Worker. Medical social services will be provided by a qualified social worker, under the direction of a physician. Social work services will be based on the patient's psycho social assessment and the patient and family's needs. The social worker participates in the coordination of care with the Interdisciplinary Group (IDG) and participates in on-call rotation as the Agency needs.
Requirements- Education:
Master's Degree in Social Work from a State accredited program. - Licensure:
Texas Certification and a current Driver's License. - Experience:
Minimum one year experience in a health care agency. Hospice experience preferred. - Skills:
Working knowledge of community resources and good interpersonal skills. - Transportation:
Reliable transportation and valid auto insurance.
