Position: Community Based Care Coordinator I - Must Reside in Crittenden County - R12081-2

Job Summary

The Community Based Care Coordinator I – Arkansas, reporting to the care coordination team, serves as the single point of contact for assessment, person‑centered planning, service coordination (funded or unfunded), disease management, and transitions between all levels of care.

Essential Functions
  • Understand and implement person‑centered thinking.
  • Facilitate the person‑centered planning process.
  • Assist with in‑person centered care training to maximize the development of the Person‑Centered Service Plan.
  • Coordinate services and health benefits for members who meet criteria.
  • Consult with members, families, and legally responsible persons about behavioral and physical health care needs.
  • Collaborate with other professionals and community members to coordinate care and develop Person‑Centered Service Plans.
  • Maintain ongoing communication with the internal complex clinical team.
  • Educate members about their condition, medication, and any necessary instructions.
  • Monitor service delivery to ensure appropriateness of care and compliance with any waiver.
  • Complete psychosocial health care questionnaires and behavioral assessments by gathering information from the member, family, provider, and other stakeholders.
  • Monitor and evaluate Person‑Centered Service Plan on an ongoing basis, modifying it as needed based on member choice.
  • Support member outcomes through coordinated care activities.
  • Maintain accurate documentation of contacts, treatment plans, case notes, referrals, and assessments in the electronic record, meeting accreditation and compliance guidelines.
  • Participate in meetings with providers to inform them of services and benefits available to members.
  • Engage members through information collection and assertive outreach, including home visits and telephone calls.
  • Educate members/caregivers about health care access and benefits, providing health education and wellness materials.
  • Travel regularly for member visits, provider visits, and community‑based visits as needed to ensure effective program administration.
  • Document care coordination activities and member responses promptly, following Care Source policies and professional practice standards.
  • Identify opportunities to improve processes, share findings with leadership to develop repeatable best practices.
  • Perform any other job duties as requested.
Education and Experience
  • High School Diploma or GED equivalent required.
  • Minimum of one (1) year of experience working with developmentally or intellectually disabled or behavioral health clients (e.g., discharge planning, case management, care coordination, and/or home/community health management).

Licensure and Certification
  • Valid driver’s license, vehicle, and verifiable insurance (conditional on clearance of a driver’s license record check).
  • Successful completion of a criminal background check (conditional for employment).
  • Successful clearance of a pre‑employment drug screen and annual drug screens thereafter (conditional for employment).
  • Annual proof of influenza vaccination required during influenza season (October 1 – March 31) for this position. Employees hired during influenza season have 30 days to complete vaccination.
