Position Overview

The Bungalows at Springdale is seeking a CNA to join their team!

The Lead Resident Care Associate reports directly to the Pearl Director.

Shift

Monday & Friday 3:00 p.m.

- 11:00 p.m.; on call required.

Purpose

The Lead Resident Care Associate provides high‑quality care and services to residents in our assisted living and memory care neighborhoods, including hands‑on care, physical and emotional support as outlined in each resident’s Personalized Service Plan (PSP). This role demonstrates wellness and follows all federal, state, local standards and Phoenix policies.

Principal Duties and Responsibilities
  • Assist in managing the incontinence program
  • Ensure resident hydration schedule is maintained during the shift
  • Know and comply with all resident rights
  • Initiate, assign, and assist with activities as appropriate
  • Check all residents daily for cleanliness, appropriate clothing, any continence issues, and changes in condition
  • Use proper sanitary procedures and universal precautions
  • Check resident rooms daily for general cleanliness and neatness and act as needed; double‑check high‑risk rooms
  • Check laundry rooms and common areas – including bathrooms – for cleanliness, exposed hazardous chemicals, and act appropriately
  • Ensure ADLs for residents are satisfactorily completed and follow up according to Phoenix policy
  • Complete charting as scheduled and check for other noted events (e.g., admissions, returns from hospital)
  • Respond promptly to security system and resident call bells; reset call bells as needed
Structure
  • Encourage active participation and teamwork across shifts and lead shift crossover meetings
  • Initiate daily RCA assignments
  • Provide assignments to RCAs
  • Monitor attendance, assign break times, and ensure proper staffing during break times
  • Review communication log regularly throughout shift
  • Assist Wellness Nurse with vital signs, if necessary
  • Train and orient new hires
  • Provide input for talent reviews, coaching, and improved performance of RCAs
Specific Responsibilities for LRCAs Assigned to Memory Care
  • Invite and assist residents to activities and life‑enriching activities as indicated on the PSP
  • Incorporate the concepts of "Creating Pleasant Days" and "Joining the Journey" into residents’ daily routines
  • Integrate individual residents’ life skills into daily routines
  • Blend elements of wellness into activities throughout the day
Requirements Education / Experience / Licensure - Certification
  • High school education preferred and may be required by the state
  • Previous experience working with elders or disabled individuals, preferred
  • Must be at least 18 years of age for personal care duties
  • Knowledge of Assisted Living industry and Dementia care or Skilled Nursing preferred
  • Ability to work weekends, evenings, and flexible hours; available at peak service delivery times

  • Show working knowledge of state regulations and Phoenix policies
  • Understand Phoenix core objective of incorporating all six elements of wellness into resident care
  • Review and understand orientation checklist
  • Possess written and verbal skills for effective communication
