additional informationthe case manager assistant works with the case managers to facilitate patient's continuity of care for home at time of discharge. The assistant is responsible for making referrals to community care agencies and communicating these arrangements with the case manager and documenting in the patient's medical record. Additional duties include assisting in writing appeal letters for denied medical stay and completing discharge follow-up phone calls.
required educationcompletion of an approved program per state's requirements.
required work experiencerelated field - 1 year of experience
required certificationslicensed practical nurse - arkansas state board of nursing (asbn)
description- complete referral to community resource agencies for patients with home discharge needs.
- coordinate and communicate the arrangement of the discharge planning activities.
- document agencies who will be providing services in the medical record along with the completion of follow-up phone calls.
- complete discharge follow-up phone calls and document in medical record. Communicate any relevant gap in providing care to a patient that is identified during the phone call.
- assist the manager in writing appeal letters for denials of in-patient admissions.
