Licensed Master Social Worker

Job Summary:

The Licensed Master of Social Work (LMSW) provides comprehensive psychosocial assessments and discharge planning for patients in a hospital setting. The role involves collaborating with multidisciplinary teams to develop safe and effective discharge plans, advocating for patient needs, and coordinating post-hospital care services. The LMSW supports patients and families through counseling, crisis intervention, and resource education to promote health and well-being in the community.

A minimum of two years of hospital experience and licensure in New York State are required.

• Complete patient assessments including biopsychosocial, financial, medical, and physical factors, respecting cultural, language, religious, and spiritual values.
• Assess patient's capacity for self-care and availability of family/friends to provide care as needed.
• Engage patients and families in developing and implementing discharge plans.
• Perform ongoing reassessments and update plans based on changes in patient condition.
• Provide post-hospital care options and coordinate placement in healthcare facilities when necessary.
• Collaborate closely with multidisciplinary teams to ensure coordinated and safe discharge planning.
• Communicate effectively with social work staff and other healthcare providers for proper hand-offs.
• Utilize community-based resources to support patient needs and maintain health in the community.
• Advocate for patients with payors and referral sources to access necessary services.
• Provide short-term counseling for problem-solving, crisis intervention, bereavement, and adjustment to medical diagnoses.
• Perform psychosocial or discharge planning evaluations upon request.
• Seek guidance for complex cases and demonstrate effective prioritization and time management skills.
• Comply with medical/legal reporting requirements including abuse reporting.
• Participate in staff meetings, trainings, and quality improvement activities.
• Adapt to changing work assignments and schedules as needed.
• Perform additional duties as assigned.

• Licensed Master of Social Work (LMSW) credential, licensed in New York State.
• Minimum of 2 years experience working in a hospital setting.
• Strong assessment and discharge planning skills.
• Excellent communication and interdisciplinary collaboration abilities.
• Knowledge of community resources and patient advocacy.
• Ability to perform crisis intervention and short-term counseling.

• Experience with Power chart electronic medical records system.

• Work assignments and schedules may be subject to change based on departmental needs.
• Must respect and accommodate patient cultural, language, religious, and spiritual preferences.

• Shift options include 8:00 AM to 4:00 PM or 9:00 AM to 5:00 PM.
