About This Role

As an RN on the Home Care Team at Cleveland Clinic, you will provide patient‑first healthcare to individuals in their homes, focusing on assessments, complex treatments, and education to enhance patient comfort and avoid readmissions. This PRN position offers flexibility to maintain a healthy work‑life balance.

• Pay: $41.00/hour
• Location: Independence, OH
• Schedule: PRN

• Provide direct nursing care in accordance with established policies and procedures.
• Implement and monitor patient care plans.
• Supervise LPNs, Home Care Aides, and family caregivers.
• Perform admission assessments and reassessments within specified time frames.
• Assess patients for safety risks, abuse, and neglect.
• Coordinate care among all disciplines for well‑coordinated patient care.
• Document patients’ pain and manage it appropriately.
• Maintain complete and up‑to‑date documentation regarding patient condition and care effectiveness.

Education: Graduate from an accredited school of Professional Nursing.

Experience: One year of clinical experience as an RN in an acute medical/surgical setting.

Licenses &

Certifications:

• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification through AHA or American Red Cross.
• Valid Driver’s License in Ohio and proof of automobile insurance with $100,000/$300,000 coverage.
