As a Care Transition Liaison, you will act as a resource for patients referred to home health or hospice, facilitating their transition between the referring physician or hospital and the LHC agency. You will educate patients and families on home health and hospice practices, ensuring the agency can meet their needs and support the Care Transition Coordinator or Account Executive within the facility.

Responsibilities
  • Facilitate referral of new or existing patients to home health agency following Right of Choice
  • Collaborate with physician and/or case manager in identifying the referred patient’s needs and obtain approval for admission from agency
  • Obtain appropriate medical record information and send to appropriate agency
  • Coordinate start of care between hospital/physician and agency
  • Under the direction of the branch, collaborate with appropriate CM and/or physician for completion of order sets needed to facilitate admission
  • If license allows, reduce written verbal orders received from physician and follow up to have orders signed by physician
  • For physician office referrals, assist with processing requests to the physician for medication refills for patients admitted to agency, when applicable, based on licensure of employee
  • Communicate to Executive Director any complaints or concerns from physician office or host hospital and ensure issues are handled appropriately
  • Serve as a resource to the CM, physician office, and patient regarding services that home health can provide
  • Document activities and productivity within available systems including but not limited to Markets cape CRM

Required Qualifications
  • Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation
Preferred Qualifications
  • LPN / LVN or licensed medical professional
  • Prior experience in hospital, physician’s office, home health, or hospice

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401(k) contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $56,012 to $84,018 annually based on full‑time employment. We comply with all minimum wage laws as applicable.
