Position Overview

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to homebound patients throughout the Springdale, Arkansas area. The RN will be responsible for conducting comprehensive patient assessments, completing OASIS documentation, developing individualized care plans, and delivering high-quality nursing interventions in the home setting. This role requires excellent clinical judgment, strong time management skills, and the ability to work independently while coordinating care with physicians, caregivers, and interdisciplinary team members.

  • Shift: 8:00 AM - 4:30 PM
  • Work Days: 5 days per week
  • Travel Requirement: Yes, daily local travel
  • Coverage: Multiple patient visits per day

  • Pay Rate: $51.00 - $53.00 per hour
Key Responsibilities
  • Conduct comprehensive nursing assessments for home health patients.
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and other OASIS assessments accurately and in a timely manner.
  • Develop, implement, and update individualized patient care plans based on clinical findings and physician orders.
  • Provide skilled nursing care to homebound patients, including disease management, medication management, wound care, and patient education.
  • Monitor patient progress, identify changes in condition, and communicate findings to physicians and care team members.
  • Educate patients and caregivers regarding diagnoses, medications, treatments, disease processes, and safety measures.
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals to ensure optimal patient outcomes.
  • Maintain accurate clinical documentation and complete all required charting within established time frames.
  • Ensure compliance with Medicare guidelines, home health regulations, agency policies, and professional nursing standards.
  • Manage a daily caseload efficiently while traveling to multiple patient homes throughout the assigned territory.
  • Participate in care conferences and interdisciplinary team meetings as needed.
  • Promote patient independence, safety, and quality of life while supporting care goals in the home environment.
Required Qualifications
  • Active Registered Nurse (RN) license with Arkansas practice privileges.
  • Home health nursing experience required.
  • Experience completing OASIS assessments and documentation.
  • Strong clinical assessment and critical thinking skills.
  • Valid driver's license, reliable transportation, and proof of automobile insurance.
  • Ability to travel locally on a daily basis.
  • Excellent communication, organizational, and time management skills.
Preferred Qualifications
  • Experience using Homecare Homebase (HCHB).
  • Extensive home health field nursing experience.
  • Strong knowledge of Medicare home health regulations and compliance requirements.
  • Previous experience managing an independent patient caseload.
  • Demonstrated ability to build positive relationships with patients, families, and healthcare providers.
