Full-time 40 hours/week
**** What You’ll Do
** Plan, organize and implement social work to patients and families. Plan for discharge and aftercare. Maintain contact with referring agencies and related people, and with community resources used for referral.
** Requirements:
*** Masters of Arts in Social Work or related field
* Preferred minimum 2 years experience
