Overview
The Illinois Department of Public Health is seeking a highly motivated, detailed oriented individual to serve as a field supervisor. This position oversees a multidisciplinary team that conducts surveys of long-term care facilities, including state licensure surveys, federal certification surveys and complaint surveys. The Field Supervisor is responsible for assigning licensure, certification and complaint surveys, reviewing written survey documentation, testifying in court and performing monitoring functions.
Responsibilities- Under administrative direction, oversees Intermediate Care Facilities/Individuals with Intellectual Disabilities (ICF/IID) facility survey activities.
- Serves as working supervisor.
- Conducts state licensure, federal certification, and complaint surveys of ICF/IID facilities.
- Develops ICF/IID facility survey schedules, reviews and processes survey packages and makes recommendations to Central Office.
- Completes necessary surveyor training.
- Represents the Division through participation in meetings with ICF/IID industry representatives, advocacy groups, and other interested parties as requested.
- Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
- Requires knowledge, skill and mental development equivalent to completion of four years college.
- Requires prior experience equivalent to three years of progressively responsible administrative experience in nursing.
- Requires licensure as a Registered Nurse in the State of Illinois.
- Three (3) or more years extensive knowledge of state and federal regulations related to long-term care facilities.
- Surveyor Minimum Qualifications Test (SMQT) certification.
- Three (3) or more years of supervisory experience.
- Two (2) years of experience establishing, maintaining, and improving cooperative working relationships with various stakeholders.
- Two (2) years of experience drafting, reviewing, and implementing policies and procedures.
- One (1) year experience coordinating assignments and schedules and prioritizing survey activities.
- One (1) year work experience in Microsoft Word, processing, and spreadsheet applications (i.e., Microsoft Word, Microsoft Outlook, Excel, Power Point, Smartsheet).
Work Hours: Tuesday-Friday 7:30AM-5:30/5:00PM (1/2 hr lunch)

Full Range $8,281 - $12,223/month
