Job DescriptionTITLE OF VACANCY: Physical Therapist (Part Time)
LOCATION: SSS/Graham (Travel)
HOURS PER DAY: 8
DAYS PER YEAR: 95 (2.5 days per week)
REQUIREMENTS/
QUALIFICATIONS:
- Bachelors, Masters degree or Doctorate (as applicable) from an accredited program for Physical Therapists
- Valid State of Illinois registration as a Physical Therapist Licensed by Illinois Department of Financial and Professional Regulation (IDFPR)
- School-based and/or pediatric experience preferred
- Identification and Planning
- Assemble, review, record pertinent information upon referral
- Maintain student files and current prescriptions
- Evaluate students to determine need for therapy and level of service
- Plan treatment programs and recommend treatment goals (IEPs)
- Service Delivery
- Provide input to multidisciplinary conferences
- Supervise programs conducted by Certified Physical Therapy Assistants (PTAs)
- Update treatment programs and goals as appropriate
- Provide direct and consultative service as appropriate
- Document evaluation/intervention procedures and results through progress notes and report
- Perform other duties as assigned
