Role DescriptionThis is a part‑time hybrid role for a Patient Advocate based in the Greater Chicago Area, with the flexibility to perform some responsibilities remotely.
Responsibilities- Conduct medication reviews and clinical assessments
- Attend medical appointments and translate medical information
- Support hospital discharges and care transitions
- Strong Communication and Customer Service skills to effectively interact with patients, families, and healthcare providers
- Experience in Case Management and the ability to coordinate comprehensive care strategies
- Critical Thinking and problem‑solving skills to assess situations and make informed decisions
- Foundational knowledge in Medicine to understand medical procedures, terminology, and patient care needs
- Empathy and an aptitude for providing emotional support to clients
- Ability to work both independently and collaboratively in a hybrid work environment
- Minimum 3 years experience in geriatrics, case management, care coordination, community nursing, or chronic disease management
- Strong clinical judgment and communication skills
- Reliable transportation for local travel
- Experience supporting aging-in-place models
- Experience with complex chronic conditions and care transitions
- Bachelor’s degree in Nursing or related field
