This is a part‑time hybrid role for a Patient Advocate based in the Greater Chicago Area, with the flexibility to perform some responsibilities remotely.

• Conduct medication reviews and clinical assessments
• Attend medical appointments and translate medical information
• Support hospital discharges and care transitions

• Strong Communication and Customer Service skills to effectively interact with patients, families, and healthcare providers
• Experience in Case Management and the ability to coordinate comprehensive care strategies
• Critical Thinking and problem‑solving skills to assess situations and make informed decisions
• Foundational knowledge in Medicine to understand medical procedures, terminology, and patient care needs
• Empathy and an aptitude for providing emotional support to clients
• Ability to work both independently and collaboratively in a hybrid work environment

• Minimum 3 years experience in geriatrics, case management, care coordination, community nursing, or chronic disease management
• Strong clinical judgment and communication skills
• Reliable transportation for local travel

• Experience supporting aging-in-place models
• Experience with complex chronic conditions and care transitions
• Bachelor’s degree in Nursing or related field
