Overview
Graham Healthcare Group is seeking a Home Health Physical Therapy Assistant for a position in Springfield, Illinois.
About the RoleAs a Home Health PTA, you will collaborate with a multidisciplinary team to provide management and delivery of patient care plans for patients throughout Springfield and surrounding areas.
Key Responsibilities- Support patients in Springfield and surrounding areas.
- Collaborate with Physical Therapists and other care team members to manage and deliver patient care plans.
- Maintain accurate charting and meet critical deadlines.
- Travel to patient homes, spending approximately 20% of the day driving.
- Associate degree in Physical Therapy from an accredited college or technical school.
- Minimum one year of experience in an acute care and/or rehabilitative setting (home‑health experience preferred).
- Valid physical therapist assistant license/registration in good standing issued by the state of practice.
- Current driver’s license and ability to travel to patient locations.
- Strong time‑management skills and ability to self‑manage schedules.
- Experience with tablets, mobile phones and EMR software.
- Up to 22 paid holiday and personal days off in year one.
