Home Health Physical Therapy Assistant

Overview

Graham Healthcare Group is seeking a Home Health Physical Therapy Assistant for a position in Springfield, Illinois.

As a Home Health PTA, you will collaborate with a multidisciplinary team to provide management and delivery of patient care plans for patients throughout Springfield and surrounding areas.

• Support patients in Springfield and surrounding areas.
• Collaborate with Physical Therapists and other care team members to manage and deliver patient care plans.
• Maintain accurate charting and meet critical deadlines.
• Travel to patient homes, spending approximately 20% of the day driving.

• Associate degree in Physical Therapy from an accredited college or technical school.
• Minimum one year of experience in an acute care and/or rehabilitative setting (home‑health experience preferred).
• Valid physical therapist assistant license/registration in good standing issued by the state of practice.
• Current driver’s license and ability to travel to patient locations.
• Strong time‑management skills and ability to self‑manage schedules.
• Experience with tablets, mobile phones and EMR software.

40 hours per week, 8‑hour shifts. Ongoing full‑time staff employment.
