Role Overview

Full‑time Physical Therapist in Milwaukee, Wisconsin. You will work in patients’ homes, helping them regain strength, mobility, and independence.

Milwaukee, Wisconsin – servicing Washington and Ozaukee counties. Flexible, territory‑based role.

• Plan and administer prescribed physical therapy treatment to patients in their homes to attain the highest level of physical function.
• Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with the physician’s plan of treatment and the patient team goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

• Degree from an accredited Physical Therapy Program approved by the APTA.
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• One year of experience as a Physical Therapist (preferred).

• $59.00 - $83.00 per visit/unit.
• $93,000 - $128,000 per year base pay.

• TB Statement:
  This role is part of Humana’s Tuberculosis screening program and requires TB screening.
• Driving Statement:
  Requires a valid state driver’s license and personal vehicle liability insurance meeting state or specified limits.
• Travel:
  Occasional travel to Humana’s offices for training or meetings may be required.
