Role Overview
Full‑time Physical Therapist in Milwaukee, Wisconsin. You will work in patients’ homes, helping them regain strength, mobility, and independence.
LocationMilwaukee, Wisconsin – servicing Washington and Ozaukee counties. Flexible, territory‑based role.
Key Responsibilities- Plan and administer prescribed physical therapy treatment to patients in their homes to attain the highest level of physical function.
- Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician’s plan of treatment and the patient team goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
- Degree from an accredited Physical Therapy Program approved by the APTA.
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- One year of experience as a Physical Therapist (preferred).
- $59.00 - $83.00 per visit/unit.
- $93,000 - $128,000 per year base pay.

This role is part of Humana’s Tuberculosis screening program and requires TB screening. - Driving Statement:
Requires a valid state driver’s license and personal vehicle liability insurance meeting state or specified limits. - Travel:
Occasional travel to Humana’s offices for training or meetings may be required. - Scheduled Weekly

40 hours.
