Position Overview

In alignment with Com Well’s Mission, Vision, and Values, the Direct Support Professional (DSP) provides direct care and support to individuals with behavioral health and/or intellectual disabilities. This role assists individuals with daily living activities, community involvement, skill development, and personal growth while promoting independence in a safe and supportive environment.

The DSP works as part of a team to provide trauma‑informed, person‑centered services that enhance the health, well‑being, and quality of life of individuals served.

Key Responsibilities
  • Assist individuals with daily living activities, including personal hygiene, grooming, dressing, feeding, and mobility
  • Support implementation of client‑centered plans and document progress toward goals
  • Monitor client health, behavior, and overall functioning and communicate changes to appropriate staff
  • Assist with behavior support plans and crisis intervention as needed
  • Provide instruction and support with life skills, social skills, money management, and community integration
  • Advocate for individuals and encourage independence and self-advocacy
  • Transport individuals to appointments, community activities, employment, and program services using agency vehicles
  • Maintain accurate documentation and compliance with agency policies and procedures
  • Participate in team meetings; trainings; and quality improvement initiatives
  • Perform other duties as assigned
Qualifications
  • Proof of 8th grade education required;
    High school diploma or GED preferred
  • DSP certification required within 120 days of hire
  • Strong communication and interpersonal skills
  • Basic computer proficiency, including Microsoft Office
  • Ability to work independently and as part of a team
  • Valid driver’s license and ability to pass DOT physical required
Physical Requirements
  • Ability to perform essential job functions in accordance with ADA, FMLA, and applicable standards
  • Ability to maintain regular and punctual attendance
  • Ability to frequently move, bend, kneel, crouch, and lift/push/pull up to 50 lbs or more

Compensation : $19.60 per hour
