Case Manager – Behavioral Health
CVS Health is seeking a Case Manager Behavioral Health to support patients with mental health and substance use needs across the care continuum. In this role, you will complete psychosocial assessments, develop person-centered care plans, coordinate services, and advocate for timely, evidence-based treatment. You will collaborate closely with providers, community resources, and internal teams to reduce barriers to care and avoid unnecessary hospitalizations.

- Conduct comprehensive behavioral health and psychosocial assessments for individuals with mental health and substance use conditions.
- Develop and maintain individualized, person-centered care plans aligned with clinical guidelines and member goals.
- Coordinate behavioral health, medical, and social services across providers, facilities, and community resources.
- Monitor progress, adjust care plans, and perform outreach to support adherence, engagement, and recovery.
- Perform utilization review/management activities in line with health plan policies and evidence-based criteria.
- Identify and address social determinants of health impacting access, safety, and stability.
- Document all interactions and care plans accurately in electronic health records and care management systems.
- Collaborate with psychiatrists, therapists, primary care, and internal CVS Health teams to ensure integrated, high-quality care.
- Provide education, advocacy, and crisis support, escalating safety risks per established protocols.
- Contribute to quality improvement, population health initiatives, and performance goals within the behavioral health program.

- Active, unrestricted clinical license preferred (LCSW, LMHC, LPC, LMFT, RN or equivalent, depending on state)
- Certification in Case Management (CCM or ACM) preferred
- Crisis intervention or de-escalation training a plus
- Experience with managed care or health plan environments preferred
- Familiarity with HIPAA and behavioral health regulations
