Overview
Jackson Therapy Partners is seeking a travel Home Health Occupational Therapist for a travel job in Springfield, Illinois.
Responsibilities- Provide skilled occupational therapy services in home health settings.
- Deliver individualized treatment plans, perform functional assessments, collaborate with interdisciplinary teams, and document progress to support patient safety and independence in the home.
- Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited educational program.
- Active Illinois Occupational Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience with Home Care Home Base EMR preferred.
- Productivity expectation: 30 visits per week for registered therapists.
- Orientation: 5 days.
- Experience:
Home health experience preferred; candidates new to the setting will be considered.
- Previous home health clinical experience preferred.
- Familiarity with home health documentation and billing practices preferred.
- Schedule:
Monday through Friday. - EMR:
Home Care Home Base. - Orientation period: 5 days.
- Productivity target: 30 visits per week for registered therapists.
