Responsibilities
The Bereavement Coordinator is responsible for planning, implementing, and maintaining a bereavement program to meet the needs of families and caregivers from Covenant Care Hospice for up to one (1) year following the death of the hospice patient.
Coordinates assessment and delivery of grief counseling needs and services (one‑to‑one, group, and ongoing follow‑up) to ensure timely, appropriate services are provided to family and caregivers.
Provides bereavement information and referral services to community callers; expands and maintains community resource list as needed.
Participates in hospice activities such as in‑service education, personnel meetings, and relevant committees.
Provides bereavement support to hospice personnel coping with work‑related grief through one‑to‑one counseling, supervision of interns providing one‑to‑one counseling, and referral to community resources.
Assists as needed in evaluating and planning services to meet hospice personnel support needs, including yearly retreats and weekly support groups.
Functions independently, while seeking guidance from interdisciplinary team members or community members.
Actively participates in quality assessment, performance improvement teams, and activities.
Education and Experience- Master’s degree in Social Work with qualifications in experienced clinical grief counseling.
- Experience working with families and caregivers and patients through death/dying issues.
- Experience in clinical grief counseling.
- Understanding of grief response and experience counseling individuals and/or groups regarding loss.
- Ability to work as a team member.
- Knowledge and commitment to hospice philosophy.
- Demonstrates good verbal and written communication, and organizational skills.
Requires mobility to move around the facility; majority of time spent sitting, walking, bending, stooping, and driving.
Must lift loads not normally exceeding medium weight (35 pounds).
Requires manual dexterity, keen vision, and accurate hearing for driving a vehicle.
CompensationHourly rate: $23.62 - $32.16 per hour.
