Overview

Are you a Physical Therapist (PT) with a desire to impact the lives of older adults? If so, our Rehab Team at Physical Therapist at Ever True Concordia Village in Springfield, IL is seeking its next full‑time, employee‑owner team member. This role is eligible for a $3,000 sign‑on bonus!

• Establish appropriate goals and creatively execute treatment plans.
• Supervise Physical Therapist Assistants.
• Create and maintain interdisciplinary care plans to meet productivity standards and complete required documentation.
• Help patients achieve critical outcomes and act as a trusted partner to the community served.

• Graduate of an accredited school for Physical Therapists.
• Hold a valid or pending state license.
• Experience in acute care, long‑term care and/or rehabilitation is great but not required.
