Location – Behavioral Health, 17 N State St., Chicago, IL 60602. Salary Range: $65,000.00 – $68,000.00 per year.

Overview

Open Roads (a department of A Safe Haven), a leading provider of comprehensive behavioral health services, is seeking a compassionate and dedicated Licensed Social Worker/Licensed Professional Counselor to join our team. Open Roads serves children, adolescents, young adults, and their families through our Intensive Outpatient Programs (IOP) and traditional outpatient therapy. We also have a partnership with a public charter school in which we provide weekly individual and/or group therapy on‑site with high school students.

This role will provide therapy services in three high schools located on the South Side of Chicago. Open Roads mission is to provide quality, evidence‑based treatment to historically disenfranchised communities and clients.

Responsibilities
  • Conduct comprehensive psychosocial assessments to evaluate clients' needs, strengths, and goals. Develop individualized treatment plans in collaboration with clients, utilizing evidence-based therapeutic modalities and interventions to address behavioral health issues.
  • Provide individual and/or group therapy sessions with youth to address mental health issues, such as mood disorders, anxiety, and substance abuse. When necessary, engage with families in consultations or therapy sessions to address family dynamics, communication, and other issues that may be impacting the adolescent’s mental health.
  • Coordinate care for adolescents, including developing treatment plans, monitoring progress, and adjusting plans as necessary. Work with families, schools, and other involved parties to ensure comprehensive care.
  • Collaborate with other mental health professionals, including psychologists, case managers, and counselors to provide coordinated care. Participate in regular team meetings to discuss cases and treatment strategies.
  • Maintain accurate and timely records of all therapy sessions, assessments, treatment plans, and communications with other professionals. Ensure that all documentation complies with state and federal regulations as well as organizational policies.
  • Assist with the development of a discharge plan when adolescents are ready to transition out of treatment. Ensure continuity of care, make referrals to appropriate services, and prepare the youth and their family for the transition.
  • Stay abreast of current research, best practices, and trends in the field of behavioral health and social work. Engage in ongoing professional development activities, including training, supervision, and peer consultation, to enhance clinical skills and knowledge.

Location:

This role is a de‑centralized position taking place in three different South Side schools from about September to June, although there may be instances during this timeframe where infrequent/occasional travel to the main clinic location (17 N. State St.) will be necessary for trainings, etc. During summer months, the goal is for school clients to transition to virtual or on‑site appointments at the clinic.

Applicants can expect to spend summer months at 17 N. State St. with the possibility for hybrid work.

Qualifications
  • Be available to be on‑site at identified schools Monday‑Friday during school hours.
  • Manage a caseload of 24–32 individual, family, and/or group appointments.
  • Master’s degree in psychology, social work, counseling, or a related field from an accredited program.
  • Current licensure as a Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) in the state of Illinois.
  • A minimum of 3 years of clinical experience providing therapy and counseling services to individuals with behavioral health disorders, preferably in a community mental health, outpatient settings, or schools.
  • Experience in working with adolescents and/or family clients.
  • Strong clinical assessment and diagnostic skills, with the ability to formulate accurate clinical impressions and treatment plans based on client needs.
