Job Description
Join a world‑class academic healthcare system, UChicago Medicine, as a Patient Care Coordinator at the University of Chicago Medical Center in Hyde Park. In this role you will work closely with our Medical Oncology teams to support exceptional patient care and coordination. This 100% onsite position requires candidates to be based in the greater Chicagoland area.
Under direct supervision, the patient care coordinator will guide patients through the healthcare system and ensure those patients and their caregivers can access culturally and linguistically appropriate services in a timely, cost‑effective manner. The coordinator will complexly coordinate care for high‑acuity patients across multiple departments, ancillary areas, and UCM Network sites. Hybrid: on‑site support at Hyde Park main campus and UCM Network sites.
EssentialJob Functions
- Serve as single point of contact with new oncology patients after initial appointment scheduling, responsible for scheduling appointments and coordinating referrals for testing, imaging, procedures, social work, physical therapy, occupational therapy, chaplain/spiritual services, geriatric‑oncology, psycho‑oncology, nutrition, palliative care, pharmacy clinic, SOCARE clinic, labs, clinical trials, and other care/support services at the hospital or community; contact patients via phone, email, and/or letter to schedule, confirm, and send reminders.
- Provide general education to patients and caregivers about the care plan, follow up at regular intervals to ensure timely appointment attendance and preparedness, and field calls or questions related to appointments, ancillary services, or other coordination matters.
- Obtain prior authorizations, medical records, slides, specimens, faxes, and other needed items for coordination of care; follow up with outside labs/ancillary services for testing results and medical records, share results with appropriate services, including submitting to Medical Records and the patient’s community physician(s).
- Initiate and prepare FMLA/Disability paperwork in collaboration with Nurse Navigator, complete relevant forms, document linkage to care services in the electronic medical record, provide information to patients/caregivers about appointments/referrals, and create EPIC notes for documentation.
- Provide patient navigation and care coordination across UCM Network sites.
- Build relationships with hospital staff, clinicians, physicians, and community partners for referral services.
- Perform other related duties as assigned.
- High School diploma required;
Bachelor’s preferred. - Minimum of two years’ experience in a high‑volume complex healthcare environment.
- Excellent telephone and personal communication skills required, as is proficiency with computer systems and programs.
- Ability to multi‑task and prioritize is crucial for success.
- Compassionate listening skills required for dealing with patient/family distress related to new diagnosis.
- Ideal candidate has strong interest and desire for extensive patient and family interaction.
- Requires experience with and high commitment to customer service.
- Uses discretion and good judgment, and maintains patient confidentiality.
- Must be able to independently and professionally interact with clinicians and personnel internal and external to UCM.

In this role, we anticipate that you will generally work approximately 40 hours per week. We anticipate that you will generally be scheduled to work the Day shift, from approximately 8:00 am – 4:30 pm and will not be scheduled for on‑call shifts. We anticipate you will be scheduled to work 75% at the Hyde Park campus and 25% Remote. The days of the week that you are scheduled will be Monday‑Friday.
You will not be scheduled to work on weekends.
