• Must hold a Doctorate degree in Physical Therapy OR a Bachelor's degree in Physical Therapy and have passed the NPTEN
• Must have graduated from an accredited school
• Minimum one year’s experience
• Current state license as a Registered Physical Therapist/ PT or be proactively in the process of obtaining licensure
• Current CPR certification

• Assess referred patients requiring rehabilitation services, providing direct patient care to evaluate medical condition, functional capabilities, limitations, and potential for rehabilitation
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under physician direction
• Set realistic and achievable goals for patients, document and record patient condition, and educate patients and families on appropriate physical therapy methods
• Develop a plan of care for each physical therapy patient
• Provide skilled physical therapy services/interventions in accordance with physician orders
• Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions
• Collaborate with all disciplines to plan and evaluate team goals for each patient
