Responsibilities
The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. The in‑home setting could be a patient’s private residence or in a patient’s room in an independent living facility. The therapist evaluates and treats patients in the most functional environment, which is the patient’s home. The therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation, makes referrals to other disciplines, implements individualized therapy programs designed to restore, reinforce, and enhance performance, and instructs, educates, and trains patients and caregivers in the skills necessary to promote independence and productivity.
The position provides a flexible schedule and work environment in various settings.
Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.Adhere to department and patient treatment schedule, timely clinical and billing documentation, and work‑related meetings.Attend and contribute to patient care, staffing conferences, and related meetings.Make referrals to other disciplines and agencies to address the comprehensive needs of patients.Consult with other rehabilitation team members to select the most appropriate therapeutic regimen for each patient.Adapt program and/or plan of care according to the patient’s individual needs.Promote a safe environment and enforce elimination of fire and safety hazards.Order supplies and equipment as necessary.Clean, maintain, identify, and report equipment repairs as necessary.Put patient service first: ensure patients and families receive the highest quality of service in a caring and compassionate atmosphere.Screens, examines, and evaluates patients, including history, systems review, application of appropriate tests and measures, synthesis of information, and interpretation of examination findings to establish a diagnosis, identify impairments, determine predicted improvement and time required, identify precautions/contraindications, and design a plan of care.Develop appropriate treatment goals and methods in collaboration with the patient and caregivers, implement the plan, and complete all related documentation.Provide therapeutic techniques in areas such as therapeutic exercise, functional training, manual therapy, airway clearance techniques, integumentary repair and protection, use of electrotherapeutic, physical agent, and mechanical modalities, health and wellness, education on chronic disease management, falls assessment and intervention, and case management.Design, fabricate, fit, and/or apply adaptive devices, and train patients in the use of adaptive devices including orthotics and prosthetics.Select appropriate activities suited to the individual’s capacity, intelligence level, and interest.Instruct patients in the skills and techniques…
