Position OverviewNomad Rehab is seeking a Full Time Home Health Physical Therapist to join our growing home health team serving Chicago’s Rogers Park, Edgewater and surrounding neighborhoods. This flexible role offers the opportunity to provide individualized care in a home-based setting while working closely with other healthcare professionals. The ideal candidate is compassionate, dependable, and comfortable working independently while collaborating with interdisciplinary care teams.
Home health experience is strongly preferred.
As an Physical Therapist, you will:
- Implement and manage individualized physical therapy treatment plans in patients' homes, modifying the plan of care as needed to achieve optimal outcomes.
- Provide therapeutic exercises, manual therapy techniques, and other evidence‑based interventions tailored to each patient's needs.
- Educate patients and caregivers on treatment plans, home exercise programs, adaptive strategies, and techniques to promote independence and safety.
- Maintain timely, accurate documentation in the electronic medical record (EMR) in compliance with Medicare, Medicaid, agency, and regulatory requirements.
- Collaborate with physicians, nurses, therapists, caregivers, the director of therapy, and other members of the healthcare team to coordinate patient care.
- Support discharge planning and facilitate safe transitions to home routines or other appropriate care settings.
- Manage an independent schedule while delivering high‑quality, patient‑centered home health services.
- Supervise Physical Therapy Assistants (OTAs) and complete required supervisory documentation in accordance with organizational policies and home health regulations.
- Adhere to agency policies, procedures, and professional standards of practice.
- Participate in staff meetings, in‑service education, and continuing professional development.
Required Qualifications
- Current Professional licensure as a Physical Therapist in the state of Illinois
- Familiarity treating geriatric, orthopedic, and post‑surgical patient populations
- CPR certification
- Valid Illinois Driver’s License with a clean driving record
- Dependable, insured vehicle
- Access to a smartphone and reliable internet connection
- Ability to use email, laptops, and electronic medical record (EMR) systems
- Strong communication and interpersonal skills
- Team‑oriented with the ability to work collaboratively across healthcare teams
- Compassionate, organized, and patient‑focused
- Comfortable working with diverse patients and families
