Position: Social Worker (LSW) - Home Health

Overview

The Home Services Social Worker identifies the psychosocial needs of patients and families through assessment. Social work interventions range from resource support identification and acquisition (including community support, financial and environmental enhancement) to short term counseling and emergent crisis intervention. Social Services are provided as part of a collaboration with interdisciplinary teams. Hours may vary depending upon census and program need.

Responsibilities

Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation. Develop a bereavement plan of care to address family member/care giver needs. Interpret and communicate patient/family faith and culture traditions. Identify and communicate when spiritual/religious beliefs may impact the physical and psychosocial care provided by other team members.

Educate patient and family members in a manner that overcomes barriers; match their learning capabilities and meet fundamental needs. Complete visit notes, orders, care plans, and financial assessments in a timely manner according to policy and timeline. Document psychosocial patient/family assessments, financial assessment, and MSW interventions within the electronic medical record.

Implement a social work plan that results in:

  • Patient/family/caregiver utilization of community resources

  • Enhanced environment for care delivery
  • Dignity for the dying patient
  • Maximized patient/family coping skills
  • Support for patient/family cultural beliefs and values

Evaluate effectiveness of the social work plan of care and modify interventions as indicated. Facilitate and support patient decisions and communication of self-determined life care decisions. Provide care according to plan of care/orders. Develop social work plan of care in collaboration with the interdisciplinary team.

Practice in a manner sensitive to the needs of patients and families. Demonstrate understanding of palliative/comfort philosophy versus aggressive/curative treatment. Identify and respond to indicators of imminent death and address patient/family needs at time of death.

Demonstrate understanding of Medicare Hospice Benefit including eligibility, qualification for admission, election process, certification, recertification, transfer, non-recertification, and revocation. Utilize Memorial Funds appropriately and submit documentation in a timely manner according to policy. Complete or assist and educate patients/caregivers on advanced directives, including living will, HCPOA, and POLST forms. Assist with transportation barriers and referrals for lack of access to food, clothing, power bills, Medicaid, Community Care, SSDI, and Elder Abuse/Neglect referrals.

Qualifications

Certifications:

  • Licensed Clinical Social Worker (LCSW) – Illinois Department of Financial and Professional Regulation (IDFPR)
  • Licensed Social Worker (LSW) – Illinois Department of Financial and Professional Regulation (IDFPR)
  • Driver’s License – Illinois Secretary of State (ILSOS) with Proof of Auto Insurance

Education:

  • Master’s Degree in Social Work (required)
  • Work Experience in hospice care
Salary Range

The compensation range for this position is $27.36 per hour – $45.69 per hour. This represents a good‑faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered to a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked, and compensation model.
