Description

This position involves providing direct services to patients and their families. This program is funded to provide care for patients/clients who do not often fit well within the standard medical model. This position will work closely with medical providers, primary care teams, and other agencies to improve patient outcomes and care. This position will work closely with the SIU Pediatric Department, their Primary Care Providers, Local Health Departments, and Community Based Organizations to provide wrap‑around services and create a foundation of trust for continued care of socially vulnerable populations.

This position is ON SITE only.

Examples of Duties

Support - Direct Patient Interaction: 100%

Explain program requirements and guidelines to the general public.

Develop community resource materials that are up to date and reflective of the local community.

Conduct comprehensive interviews and intake assessments to determine the needs of individuals to be served by the program.

Assist patients in setting and achieving personal goals in person, by phone or in group setting.

Through skill building interventions and other supportive interventions, assist patients in effectively utilizing a wide spectrum of resources, managing symptoms of their illness, and developing daily living skills to gain greater independence, employment and other meaningful activities.

Connect patients to community resources including but not limited to housing, medical care, education, vocational training, and additional recovery resources / meetings.

Assist patients with transportation to medical and other related appointments.

Prepare and maintain written records as needed by the program.

Participate in all required administrative and training meetings in order to ensure adherence to policies and protocols.

Complete other duties as assigned.

Perform basic patient education on specific topics (lifestyle behaviors, medications, etc.).

Serves as designated backup van driver, providing transportation coverage as needed to ensure continuity of services.

Qualifications

Credentials to be Verified by Placement Officer

Coursework in social work or behavioral sciences, or closely related field as measured by conversion: 30 semester hours equals one year (12 months), Associate’s Degree (60 semester hours) equals eighteen months, 90 semester hours equals two years (24 months), Bachelor’s Degree (120 semester hours) equals three years (36 months).

Progressively more responsible work experience in social service/ counseling at a paraprofessional level under qualified supervision.

(KSAs)

Working knowledge of human nature.

Ability to interview individuals to determine their physical, social, and/or possible psychological needs.

Ability to organize, prepare, and maintain records, reports, files, and correspondence.

Ability to obtain services necessary to meet participants' needs.

Ability to establish effective working relationships with program participants and staff.

Pursuant to the State Universities Civil Service System, an out‑of‑state resident who is hired into this position must establish Illinois residency within 180 calendar days of their start date.
