Position Summary

Community Health Workers (CHW) work field‑based in Madison, St. Clair, Macoupin, and surrounding counties, serving as a bridge between community members and the healthcare system through outreach and education. The role supports Aetna members in attaining and maintaining better health outcomes, improving relationships with providers, and facilitating culturally appropriate care. The position requires up to 70% travel, flexibility in varying environments, and effective communication with healthcare professionals.

• Serve as a bridge between community members and healthcare providers through outreach, education, and care coordination.
• Assist members in achieving and maintaining better health outcomes by identifying and referring to appropriate community resources.
• Communicate medical information to healthcare professionals and care coordinators over the phone.
• Travel to multiple office and home locations, including up to 75% of the time.
• Influence medical care providers to adjust interventions based on members’ cultural, ethnic, or religious preferences.
• Work collaboratively in a team, manage multiple priorities, and exercise sound judgment and effective time‑management skills.
• Provide support care to socially and medically complex patients in non‑traditional settings.
• Travel up to 70% of the time, with occasional weekend hours as required.

• Must reside in Madison, St. Clair, Macoupin, or surrounding counties and possess reliable transportation.
• Must be willing and able to travel up to 75% of the time from the local area.
• 1–3 years of relevant experience (e.g., community outreach or home visits to members/patients).
• Experience serving Medicaid populations in urban or rural environments with familiarity of local resources.
• High School Diploma or G.E.D.
• Experience using a computer, including Excel and Electronic Health Records.
• Must meet all local state requirements (e.g., first aid, CPR, TB screening).

• Experience working in a community or outpatient setting.
• Community Health Worker certification or equivalent training and experience.
• Training in Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized medical terminology.

Full time

40 hours

$18.50 – $38.82 per hour. Pay is based on experience, education, geography, and other factors. Eligible for bonus, commission, and short‑term incentive programs.
