Salary Range: $30,000.00 - $30,000.00 per year

This is a year-long, full-time Ameri Corps position requiring completion of a minimum of 1,700 service hours. The work schedule for this position is aligned with Erie's hours of operations.

Main

Duties & Responsibilities
  • Provide one-on-one health education and self-management support to patients
  • Participate in community events, training, and Ameri Corps service activities
  • Support Lending Hands for Life HIV programming and re-engagement efforts
  • Conduct outreach and engagement for patients requiring additional support
  • Document patient interactions and coordinate follow-up care activities
  • Facilitate health education groups and recruit participants for programs

Education
  • High school diploma or equivalent
Skills and Knowledge Required
  • Strong verbal and written communication skills
  • Ability to work with diverse populations
  • Current CPR Certification
  • U.S. citizen, national, or lawful permanent resident
  • Ability to meet Ameri Corps background check requirements
Preferred
  • Experience in community health, social services, or related fields
  • Intermediate Microsoft Office skills
  • Experience with electronic health record systems
  • Bilingual English/Spanish skills
