PositionSummary
Conducts routine care coordination, support, and education through the use of care management resources in order to facilitate appropriate healthcare outcomes for members. Helpsimplement projects, programs, and processes for Case Management. Applies practical knowledge of Case Management to administer best of class policies, procedures, and plans for the area.
Required Qualifications- Minimum 2 years of case management experience.
- Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy. Travel required in LaSalle and Bureau Counties, IL
- Case management and discharge planning experience preferred
- Managed Care experience preferred
- Microsoft Office experience preferred
- Bachelor's Degree or Non-Licensed Masters Degree in a behavioral health or human services field (psychology, social work, counseling and/or marriage & family therapy)

Pay RangeThe typical pay range for this role is:
$21.10 - $44.99
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
