Cook County Health (CCH) seeks a CCHIP Sexual Health Clinic Coordinator to deliver HIV counseling, testing, PrEP education, assessments, and referrals in the Sexual Health Clinic and in community settings. The role supports initiation to adults and adolescents under supervision of the Manager of Patient Centered Care.
Responsibilities include developing patient education programs, coordinating PrEP care, supervising health education staff, and maintaining data and reports for funders.
