Sevita is seeking a compassionate Case Manager to support individuals in our community-based healthcare programs. In this role, you will conduct person-centered assessments, develop individualized service plans, and coordinate medical, behavioral, and social supports to help people reach their goals. You will connect individuals and families with community resources, monitor progress, and advocate for needed services. Working in a collaborative, inclusive environment, you'll partner with interdisciplinary teams, maintain accurate documentation, and participate in ongoing training to grow your skills and impact.
Responsibilities
- Conduct person-centered assessments and develop individualized service plans
- Coordinate medical, behavioral, and community-based services and supports
- Monitor participant progress and adjust care plans as needed
- Advocate for individuals and families to access appropriate resources
- Maintain accurate, timely documentation in accordance with regulations
- Collaborate with interdisciplinary teams and community partners
- Provide education and support to individuals, families, and caregivers
- Respond to emerging needs and assist with basic crisis coordination
- Ensure services align with Sevita's quality and compliance standards
- Participate in ongoing training and professional development
