Overview

Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on‑call or weekends)! We are seeking a full‑time (40 hours per week) provider for our Allure of Pinecrest Center located in Mt. Morris, IL. This position can be filled by a nurse practitioner or physician assistant. Salary range $115,000–$125,000 per year. Align Med Partners is a national medical group dedicated to improving health outcomes among residents of skilled nursing and senior living facilities.

Responsibilities
  • The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or Area Clinical Practice Manager.
  • Collaborates on‑site with Center Leadership Team for center‑specific goals and priorities.
  • Works collaboratively with Medical Director and/or Attending Physician for patient‑specific medical care.
  • Provides primary and urgent care services to nursing center residents in collaboration with other medical providers.
  • Works team‑wise to achieve Align Med Partners mission of improving lives through high‑quality health care and compassion.
Qualifications
  • Nurse Practitioner:
    Graduate from an accredited School of Nursing, licensed RN, certified NP, Master’s Degree in Nursing with concentration in Older Adult/FNP. Will consider other concentrations based on experience.
  • Physician Assistant:
    Graduate from ARC‑PA accredited program, passed PANCE, PA state licensure, NCCPA certification.
  • Minimum two years experience working with adult or geriatric population preferred.
  • APRNs with unrestricted professional license and DEA (for practicing states) also considered.
  • PA with unrestricted professional license and DEA also considered.
