Responsibilities- Assess, plan, and coordinate telephonic and virtual care for members.
- Develop proactive care plans to improve short‑ and long‑term health outcomes.
- Use clinical judgment and data insights to address complex medical and social needs.
- Support smooth transitions across care programs and benefits.
- Collaborate with multidisciplinary teams to remove barriers and optimize care.
- Engage members through effective interviewing and education to support wellness.
Required
- Active Illinois RN license (Illinois residency not required).
- 2+ years of RN clinical experience (all specialties considered).
- Associate Degree in Nursing (ADN).
- Willingness to obtain additional state licenses if selected (company‑paid).
Preferred
- Certified Case Manager (CCM).
- BSN.
- Experience in case management, managed care, or discharge planning.
- Comfort with Microsoft Office tools.
Fully remote (no travel required). Standard hours:
Monday–Friday, 9:00AM–5:30PM local time. Occasional evening, weekend, or holiday shifts as needed.
The typical pay range for this role is: $66,575.00 – $. The actual base salary offer will depend on experience, education, geography, and other factors.
