Hospice After Hours LPN (JR# JR255685)

Graham Healthcare Group (a subsidiary of Graham Holdings Company) is seeking a Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVV) to provide hospice care in Springfield, Illinois and surrounding areas. The role is ongoing, full‑time (40 hours per week) with 8‑hour shifts and includes after‑hours care team support.

This position supports patients in Springfield and surrounding areas. The LPN works with the after‑hours hospice team to manage and deliver patient care plans, ensuring timely charting and adherence to critical deadlines.

• LPN license in the state of employment
• Graduate from an approved school of practical nursing
• Minimum two years of LPN experience, preferably in hospice care
• Current driver's license and ability to spend roughly 20% of the day driving to patient locations
• Commitment to consistently meet charting deadlines
• Ability to self‑manage time and schedule
• Experience with tablets, smartphones and electronic medical record (EMR) software
