The after‑hours RN coordinates and provides direct and indirect patient care using the nursing process to meet physical, psychosocial, environmental, and spiritual needs of Carle Home Services specific patient populations and families. The role includes processing referrals, completing admissions, and providing continuity of care through triaging phone calls and visits during hours the hospice office is closed.
Qualifications- Certifications:
Proof of Auto Insurance, Illinois Driver's License, Registered Professional Nurse (RN), Basic Life Support (BLS) within 30 days - Education:
Associate's Degree Nursing - Work Experience:
1 year nursing experience
- Provide on‑call coverage and perform patient visits as assigned.
- Conduct visits and consultations for patients, families, and health care providers regarding hospice services and end‑of‑life treatment choices.
- Perform admission processes, assess referrals, and verify medical needs and prognosis.
- Maintain clinical records in accordance with established policies, state and federal regulations, and accreditation standards.
- Coordinate patient care needs during evenings, nights, holidays, and weekend hours.
- Communicate scope of hospice care to patients, families, and providers.
- Collaborate with attending physicians to confirm orders and obtain necessary consents.
- Coordinate delivery of medications, durable medical equipment, and supplies.
- Participate in the QA/CQI process as required.
The compensation range for this position is $33.62 per hour to $57.83 per hour. Actual compensation offered will depend on experience, qualifications, location, training, licenses, shifts worked, and compensation model. Carle Health offers a comprehensive benefits package.
