Job Description

Responsibilities:

• Collaborate with the health care team in planning for patient care across the continuum.
• Utilize critical thinking through prioritizing and managing complex patient care.
• Delegate tasks appropriately while maintaining accountability for outcomes.
• Communicate with the healthcare team members, patient, and care partner based on their level of comprehension and understanding.
• Demonstrate the knowledge and skills necessary to provide age-appropriate care by identifying physical, psychosocial and developmental needs and adapting interventions accordingly.
• Provide patient education and instructions at a level of understanding appropriate to the comprehension level of the patient and/or their care partner.
• Administer medications and carry out orders given by physician.
• Utilize the EMR to appropriately document patient care provided, and patient assessments completed

Must Haves:

• License: IL RN
• Certification: CPR/BLS from American Heart Association or American Red Cross.
• CPI certification required w/in 1 year of hire.
• Experience:
  
  At least 1 year of recent acute RN Med/Surg experience.
• Education:
  
  Associates Degree or higher in Nursing.
• If Associates Degree in Nursing is the highest degree held, must graduate with a Bachelor’s Degree in Nursing within five years of position start date (effective September 1, 2021).

Shift: Day shift--7a-730p or Night shift 7p-730a

Beds: 62

Common Diagnoses: Diabetes, Renal disease, Wounds, pulmonary disease, GI disorders, COVID, Substance abuse, Urinary disease

Patient Ratio: day 1:5 night 1:5-6
