Description

This position serves an expanded nursing role working in collaboration with Family Medicine providers that provide care to patients in the outpatient clinic setting. The primary focus is on pediatric patients and collaboration with foster care parents, DCFS caseworkers, and the Child Advocacy Center.

The role supports the care team in Kids Health Harbor Foster Care clinic and Child Abuse clinic, coordinating care and ensuring quality, efficient, cost‑effective services. The position involves a high level of interaction, coordination, and education. This position is ON SITE only.

Examples of Duties
  • Work under the direction of Faculty providers at Center for Family Medicine to provide nursing support and assist with scheduling patients in Kids Health Harbor and Child Abuse clinic.
  • Serve as an extension of Faculty and Resident providers at CFM, establishing rapport with patients, caregivers, community agencies, DCFS caseworkers, and patient advocates to ensure quality of care.
  • Provide patient and caregiver education as necessary and communicate care needs with the provider and care team.
  • Attend monthly meetings with faculty members at CFM that provide care for patients in Kids Health Harbor and Child Abuse clinic to gain understanding of and participate in patient care and treatment.
  • Assist the team proactively identifying care needs or linking with applicable services to facilitate quality of care. Identify patients at higher risk for care gaps or complications and assist with coordination efforts to break down barriers, implementing a patient‑centered nursing plan that promotes health improvements.
  • Coordinate and manage referrals, reporting, and information sharing with DCFS, Child Advocacy Centers, and other multidisciplinary teams while ensuring compliance with HIPAA, mandated reporting laws, and SIU Policies.
  • Provide telephone triage for incoming calls from patients, pharmacies, and other health care agencies.
  • Attend meetings with Child Advocacy Center to facilitate scheduling and communication.
  • Support patient needs, identify barriers, provide support and education, and connect with services necessary to follow recommended plans of care.
  • Assist with rooming patients, vital signs, medication reconciliation, and assist providers during visits, providing necessary patient education at the end of the clinic visit. Ensure follow‑up appointments are scheduled. Actively involved in patient education related to their care.
  • Review and coordinate necessary immunization needs for pediatric patients.
  • Follow‑up on overdue orders to ensure appropriate testing is completed and maintain quality care.
  • Work directly with the care team to ensure quality of care delivered.
  • Provide education to staff focused on care of patients in foster care, pediatric immunization, anticipatory guidance, and caregiver education.
  • Serve as a Pediatric Vaccine Lead in the clinic and provide support to providers and staff as needed.
  • Serve as clinic lead for school physical clinics by providing support and education to staff and providers.
  • Coordinate follow‑up care for children from birth to 5 years to avoid gaps in care.
  • Maintain accurate, up‑to‑date documentation for all case management activities.
Administrative/Teaching (20%)
  • Provide annual and monthly orientation for residents and students regarding care of pediatric patients. Assist with annual education regarding school physical and pediatric well‑child care.
  • Assist in collection and analysis of data for quality improvement projects.
  • Keep accurate records for patients evaluated for child abuse and ensure documentation is accurate and timely.
  • Participate in internal quality improvement teams.
  • Participate in quality improvement committees in the Center as well as externally through the school and hospital when needed.
  • Work with the health care team to continuously evaluate processes, identify problems, and propose improvement strategies to enhance the delivery of the care model.
