Pay Range
Pay Range: $35.92 - $53.88 per hour. Actual pay will be based on experience, skills, training, certifications, and education.
Role OverviewThe Care Manager‑RN collaborates with patients, families, social workers, physicians, the interdisciplinary team, and payer representatives to ensure efficient, cost‑effective progression through the continuum of care. Responsibilities include initiating and managing optimal care coordination and discharge planning, improving patient flow and throughput, ensuring smooth transitions and patient safety, and monitoring cost‑effective care delivery. The role also ensures compliance with regulatory requirements and organizational performance standards.

- Associate’s Degree or Diploma in Nursing (required)
- Bachelor’s Degree in Nursing (preferred)
- Minimum 2 years of clinical experience (required)
- Prior case management experience (preferred)
- Quality/Process Improvement experience (preferred)
- Registered Nurse (RN) license in state of practice (required)
- Case Management certification (ACM, CCM, RN‑BC) (preferred)
- Basic Life Support Certification (BLS) within 60 days of hire (required)
