Certified Nursing Assistant – Hospice

We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Springfield, IL area and is eligible for a $2,000 Sign‑on Bonus.

• Deliver compassionate, patient‑centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Provide hands‑on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life.
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey.
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the electronic medical record.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

• Active Certified Nursing Assistant (CNA) certificate in the state you work.
• High School Diploma or equivalent certificate.
• Minimum one year healthcare experience; hospice experience preferred.
• Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations.
