Signature HealthCARE is seeking a Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you will provide direct nursing care, administer medications, monitor resident conditions, and collaborate with an interdisciplinary team to support individualized care plans. You'll deliver compassionate, resident-centered care in a long-term and post-acute setting, ensuring dignity, safety, and comfort. Responsibilities
- Provide direct, resident-centered nursing care under the supervision of an RN and in accordance with state LPN scope of practice.
- Administer medications and treatments safely and accurately, following provider orders and facility protocols.
- Monitor, record, and report vital signs, changes in condition, and responses to treatments to the RN and provider team.
- Perform wound care, dressing changes, catheter care, blood glucose monitoring, and other delegated clinical procedures.
- Assist residents with activities of daily living (ADLs), promoting dignity, comfort, and independence.
- Document assessments, care, and interventions in the electronic medical record in a timely and accurate manner.
- Participate in the development, review, and implementation of individualized care plans with the interdisciplinary team.
- Support and guide CNAs and other support staff to ensure consistent, high-quality care delivery.
- Educate residents and families regarding medications, disease management, and care routines.
- Adhere to infection control, safety, and regulatory guidelines, contributing to a clean and safe care environment.
- Participate in shift report, interdisciplinary meetings, and quality improvement initiatives focused on continuous improvement.
