The Delta-T Group has a client seeking a Licensed CWEL Case Manager in the Naperville area! The Case Manager will hold a caseload of roughly 9 clients and coordinate and/or provide services and activities for youth in a group home setting, including medical, school, and other relevant appointments and family visitation. This position acts as a liaison between the Agency and other agencies such as DCFS and court, and collaborates with staff toward implementing permanency plans.

Monday‑Friday 9:00 am – 5:00 pm with required flexibility.

• Carry a client caseload with an emphasis on meeting the environmental and psychosocial needs of the client.
• Provide casework and counseling services to youth and their parents/guardians regarding permanency plans.
• Arrange and/or supervise visits with parents/guardians and families.
• Complete data entry of client information into the client management system.
• Consult and review cases with Supervisor and other team members.
• Prepare for and attend court hearings, administrative case reviews, and prepare reports and provide testimony as required.
• Participate in Continuous Quality Improvement initiatives and activities.
• Complete other duties as assigned to fulfill the mission of the Department and the Agency.
• Participate in team meetings, case reviews, staffing, and related activities.
• Collaboratively evaluate and plan for treatment when problems exist.
• Cooperate with medical staff during examinations for consultation and staffing of children.

• Must hold a CWEL License in IL.
• Bachelor's degree in Human Services or related field.
• One to three years of relevant experience.
• Passing of all DCFS required exams and certifications, including DCFS Child Welfare Licensure.
• Driver approval standards, plus insured automobile that meets safety and drivability criteria.
