Social Services Coordinator

Springfield Suites Rehab & Nursing Center is seeking a full-time Social Services Coordinator to join our skilled nursing team.

In this role, you will oversee social services, admissions support, and discharge planning while serving as a key resource for residents and their families throughout their stay.

Salary: $45,000 per year

• Complete social services assessments, including BIMS, PHQ-9, and trauma screenings
• Review admission documentation for appropriateness of placement
• Coordinate discharge planning and aftercare services
• Arrange home care, medical equipment, follow-up appointments, and community resources
• Communicate with residents and families regarding concerns and care needs
• Coordinate care plan meetings
• Maintain advanced directives and resident records
• Document assessments and services in Point Click Care

• Experience in social work, admissions, and/or discharge planning in a nursing home or long-term care setting required
• Knowledge of the Medicaid process required
• Degree in Social Services, Human Services, Social Work, or a related field preferred
• Strong communication, organization, and problem-solving skills
