seeking a Primary Care Physician to join its Springfield PACE Center as part of a mission-driven, interdisciplinary model of care. This physician will provide comprehensive primary care to frail older adults who qualify for nursing-home-level care while supporting their ability to live safely and independently in the community.
This role is ideal for a physician who values quality over quantity, thrives in a team-based environment, and is motivated by longitudinal, relationship-driven care.
Clinical Model & Environmentoperates under the PACE (Program of All-Inclusive Care for the Elderly) model, a nationally recognized, value-based approach designed to fully integrate medical care, social services, rehabilitation, and insurance coverage under one coordinated care team.
Physicians practice alongside a highly engaged Interdisciplinary Team (IDT) that includes:
- Nurse Practitioners and Registered Nurses
- Social Workers
- Physical and Occupational Therapists
- Home Care and Transportation Coordinators
- Life Enrichment and Support Staff
Care is delivered primarily through the PACE Center, with additional services provided in participant homes and skilled nursing facilities as clinically indicated.
Key Responsibilities- Provide primary medical care to participants across clinic, home, and SNF settings
- Conduct comprehensive geriatric assessments and ongoing longitudinal care
- Collaborate daily with the IDT to develop and adjust individualized care plans
- Coordinate specialty care and hospital transitions as needed
- Engage participants and caregivers using a whole-person, patient-centered approach
- Participate in quality improvement initiatives and clinical collaboration with peers
- Maintain timely, accurate documentation within the Next Gen EMR
- Average daily visits: 2–5 patients
- Daily structure:
- Clinic visits typically begin mid-morning
- Afternoon time allocated for documentation, care coordination, and family engagement
- Call: No after-hours call; first call is managed by NP/RN team
- Salary range: $275,000–$300,000 annually, based on experience and qualifications

- MD or DO with eligibility for licensure in Massachusetts
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
- DEA certification
- Minimum of one year of experience caring for geriatric or medically complex populations
- Strong alignment with team-based, interdisciplinary care
- Interest in mission-driven medicine focused on keeping elders in the community
