About the Position

Position Overview
We are seeking a  Board-Certified Internal Medicine or Family Medicine Physician  to serve as  Medical Director  for a geriatric-focused program in  West Springfield, Massachusetts .

This leadership role centers on comprehensive, community-based care for frail and elderly patients within a PACE (Program of All-Inclusive Care for the Elderly) model. The Medical Director will oversee clinical operations, ensure high-quality patient outcomes, and supervise medical staff in a collaborative interdisciplinary environment.

Schedule:

Monday – Friday, Full-Time

Leadership Role:  Reports to Executive Director

On-Call:  Participation required as part of provider rotation

Compensation:  Inquire for details

Practice Setting

Community-based geriatric care model (PACE)

Focus on adults 55+ requiring nursing-home-level care but living in the community

Interdisciplinary team approach

Emphasis on prevention, chronic disease management, and maintaining independence

Key Responsibilities

Provide medical direction and supervision of participant care

Oversee clinical outcomes and Quality Assurance Performance Improvement (QAPI) program

Develop and maintain medical policies, procedures, and standards

Supervise Primary Care Providers (PCPs) and Advanced Practice Providers (APPs)

Conduct new patient histories and physicals, reassessments, and treatment planning

Participate in interdisciplinary team meetings and family conferences

Manage chronic disease care for frail older adults

Order and interpret diagnostic tests and labs

Educate participants and families on health maintenance and care plans

Participate in intake evaluations and eligibility assessments

Requirements

Required Qualifications

Active Massachusetts Physician License – Required

Board Certified in Internal Medicine or Family Practice – Required

DEA Registration – Required

Previous supervisory experience – Required

Minimum one (1) year experience working with frail or elderly populations – Required

Self-Query NPDB (dated within the same month as submission) – Required

Preferred:

Board Certification in Geriatrics

Additional experience or qualifications in geriatric medicine
