OverviewThe Family Medicine Practitioner functions independently to provide primary medical and ambulatory care to patients of all ages at the Caring Health Center (CHC). The practitioner actively participates in the development and execution of the CHC’s adult and geriatric programs, medical community outreach, quality assurance, research, and administrative tasks as directed by the Chief Medical Officer. Additionally, the practitioner engages in the general activities of all health care providers at CHC.
Minimum Requirements- Unrestricted Massachusetts medical license
- Completion of an M.D. degree
- Three years of post‑graduate residency training in an accredited Family Medicine residency program
- Board eligibility in Family Medicine
- Current DEA and Massachusetts Controlled Substances registrations
- Demonstrated ability to provide quality care in a high‑volume, multicultural health center setting
- Unrestricted admitting privileges at Baystate Medical Center and Mercy Medical Center, maintained during tenure
- Provider status with major insurers
- Board Certification in Family Medicine
- Fluency in Spanish, Russian, or Vietnamese
- Special expertise in providing health care to underserved, culturally diverse populations
- Provide direct patient care services within the area of expertise
- Assess, plan, document, and evaluate patient needs and treatments
- Ensure appropriate follow‑up and maintain a culturally and linguistically sensitive practice, utilizing interpreters when needed
- Design a plan of care based on assessment of physical, emotional, and social needs in collaboration with the patient and family
- Refer patients to specialists, practitioners, and facilities as appropriate
- Review patient charts and ensure adherence to treatment plans
- Collaborate with other caregivers on patient education and rehabilitation, promoting mental and physical health
- Participate in on‑call rotations and support CHC administration efforts to monitor and improve patient care quality
Demonstrates sensitivity and responds appropriately to verbal and non‑verbal communications of patients, families, and co‑workers. Maintains patient confidentiality and dignity, exercising discretion in all written and oral communications about patient care. Utilizes tact, courtesy, patience, and discretion when interacting with others.
Problem ResolutionConsistently applies good judgment to resolve clinical and non‑clinical problems. Acts independently and is prepared to make decisions using sound judgment. Visualizes and anticipates new problems and participates in problem‑solving processes.
Career DevelopmentAttends and actively participates in scheduled staff meetings and professional staff teaching—both formal and informal—as required. Maintains clinical competence through ongoing medical education and training. Maintains associate privileges at local hospitals (Baystate, Mercy).
Qualifications- Doctorate (M.D.) or higher
