Northeast Center for Youth and Families, Inc. (NCYF) is hiring two full‑time Clinical Case Managers/Educational Liaisons (CCM) in Residential Services. The role focuses on clinical, collateral case management, ongoing assessments, and aftercare for youth. The CCM will also serve as the Educational Liaison, supporting youth learning and ensuring their IEPs meet the needs in the school environment.
ResponsibilitiesThe CCM will advocate for youth within the educational setting, maintain availability during school hours, attend school meetings, and be present during after‑school hours to establish relationships with students and address emerging needs. After‑hours on‑call support to programs is also required.
Qualifications- Master's Degree
- Successful completion of required trainings once hired, including but not limited to CPI, CPR, First Aid
- Approved CORI & DCF background checks (performed by the EEC)
- Approved driving background check
- Valid driver's license and reliable transportation required
