State Home Care Team Supervisor

Overview

Full-time 35 hours per week

NOT remote

Responsible for administrative, technical and casework supervision of the State Home Care Case Managers (CMs) assigned to them and to the Home Care Program.

• Process client/applicant files developed by CMs for appropriateness and approval of recommended service plans.
• Ensure technical compliance with regulations and procedures.
• Authorize client service plans with the contracted providers.
• Provide guidance to staff on issues related to safeguarding elders in the community.
• Conduct monthly individual and team supervisory sessions with case managers.
• Participate in the development of procedures to ensure program consistency and regulatory compliance.
• Oversee the development and/or training of team members.
• Develop action plans to address deficiencies and participate in disciplinary actions as needed for team members.
• Maintain and enforce strict confidentiality standards and ensure the security of client information.
• Attend individualized, supervisory sessions, in-service trainings, and meetings as necessary and assigned.
• Provide support and back up to other SHC Supervisors/Teams as needed.
• Represent the agency within the community and the service system.
• Design, implement, and manage new projects as assigned.

• Bachelor s degree in a related field required
• Requires relevant experience of over 1 year
• Supervisory experience is preferred
