$500 sign on bonus ($250 at the completion of first week of training and $250 after successfully completing first 90 days).
Position SummaryResponsible for gathering data, documenting and presenting each member’s goals and objectives in accordance with ISP regulations; coordinating services with case managers and parents/guardians of program member’s goals and objectives, issues, concerns and abilities; ensuring the provision of high quality services; overseeing the screening and prioritizing of referrals; documenting member progress; implementing behavior management strategies; investigating incidents and all other duties as assigned.
Responsibilities- Gather data, document and present each member’s goals and objectives in accordance with ISP regulations.
- Coordinate services with case managers and parents/guardians of program member’s goals and objectives, issues, concerns and abilities.
- Ensure provision of high quality services.
- Oversee screening and prioritizing of referrals.
- Document member progress.
- Implement behavior management strategies.
- Investigate incidents and perform all other duties as assigned.
- High School diploma with 5 years related experience.
- Associate’s degree with three years related experience.
- Bachelor’s degree with one year related experience.
- Knowledge of and respect for diverse populations.
- Previous experience working in human services preferred.
- 2 years of previous case management or care management experience required.
- Ability to use computers, including Microsoft Word, data entry, email, and internet essential.
- Credible experience preferred.
- Must be organized, detail-oriented, have solid time management skills, and be able to work in a fast-paced, multi-tasking environment.
- Demonstrated ability to deliver high level of customer service/satisfaction with both internal and external customers.
- Excellent communication, interpersonal, and listening skills.
- Must have a valid driver’s license, good driving record, use of vehicle, and willingness to transport participants in personal and/or agency vehicle.
