Rehabilitation Specialist - Full Time

Springfield, MA | 40 hrs per week | Pay: $26.00 per hour

Bonus

$500 sign on bonus ($250 at the completion of first week of training and $250 after successfully completing first 90 days).

Position Summary

Responsible for gathering data, documenting and presenting each member’s goals and objectives in accordance with ISP regulations; coordinating services with case managers and parents/guardians of program member’s goals and objectives, issues, concerns and abilities; ensuring the provision of high quality services; overseeing the screening and prioritizing of referrals; documenting member progress; implementing behavior management strategies; investigating incidents and all other duties as assigned.

Responsibilities
  • Gather data, document and present each member’s goals and objectives in accordance with ISP regulations.
  • Coordinate services with case managers and parents/guardians of program member’s goals and objectives, issues, concerns and abilities.
  • Ensure provision of high quality services.
  • Oversee screening and prioritizing of referrals.
  • Document member progress.
  • Implement behavior management strategies.
  • Investigate incidents and perform all other duties as assigned.
Qualifications
  • High School diploma with 5 years related experience.
  • Associate’s degree with three years related experience.
  • Bachelor’s degree with one year related experience.
  • Knowledge of and respect for diverse populations.
  • Previous experience working in human services preferred.
Requirements
  • 2 years of previous case management or care management experience required.
  • Ability to use computers, including Microsoft Word, data entry, email, and internet essential.
  • Credible experience preferred.
  • Must be organized, detail-oriented, have solid time management skills, and be able to work in a fast-paced, multi-tasking environment.
  • Demonstrated ability to deliver high level of customer service/satisfaction with both internal and external customers.
  • Excellent communication, interpersonal, and listening skills.
  • Must have a valid driver’s license, good driving record, use of vehicle, and willingness to transport participants in personal and/or agency vehicle.
