Arbor Associates is seeking a Licensed Clinical Care Coordinator to join a respected, mission-driven behavioral health program serving adults throughout the Westfield area. This is more than a traditional care coordination position. You will provide clinical direction to an integrated community-based team, develop individualized treatment plans, and help adults living with serious mental health challenges work toward greater stability, independence and community connection.
You will have the opportunity to combine direct clinical work with team-based leadership - guiding interventions, coordinating services and ensuring that each person receives thoughtful, recovery-oriented support.
- Oversee the clinical services provided by an assigned integrated ACCS team and associated group living environments.
- Complete comprehensive assessments and develop individualized, person-centered treatment plans.
- Provide direct clinical interventions to adults with complex behavioral health needs.
- Coordinate care alongside individuals, families, clinicians, medical providers, peer specialists and community partners.
- Offer clinical guidance, modeling and oversight to outreach and residential direct-care staff.
- Ensure that team interventions remain consistent with each individual’s treatment plan.
- Perform clinical responsibilities designated for a Licensed Practitioner of the Healing Arts, or LPHA.
- Support individuals with needs involving mental health, physical health, substance use, housing, employment, daily living and community integration.
- Maintain accurate, timely documentation that meets DMH and ACCS requirements.
- Promote trauma-informed, evidence-based and recovery-focused practices throughout the program.
This position offers the chance to make a meaningful impact outside of a traditional office-based setting. You will help individuals strengthen their daily living skills, navigate complex systems, connect with community resources and move toward goals that matter to them. Through Arbor Associates, you will join a collaborative partnership with an established Western Massachusetts behavioral health provider known for accessible, community-based mental health and recovery services.
The ACCS model is designed to reduce reliance on hospitals and other institutional settings by delivering coordinated clinical, rehabilitative and peer support where people live.
